Military Discharge

To obtain a certified copy of discharge records you must:

  • Have Recorded Discharge Paper

  • The person who is subject of the record

  • A family member or legal representative of the person who is subject of the record

  • County office that provides veteran's benefits upon written request of that office

  • United States Official upon written request of that official

Step 4: Pick Up at Second Floor

By Mail

Step 1: Print & fill out Application for Certified Copy of DD-214(PDF, 261KB)

Step 2: Get documents Notarized 

Step 3: Include Self Envelope with Address to send back

Step 4: Mail to P.O. Box 766 Fresno, CA 93712


What you need to provide:

If you order in person

  • Name of veteran

  • Complete sections 1 through 4 on the form. Your signature must be witnessed by a clerk in our office. It does not need to be notarized.

  • Photo ID required

  • No charge

     

 

If you order by mail

  • Name of veteran

  • Complete sections 1 through 5 on the form. Your signature must be notarized.

  • The name and address of the person or agency to whom the copies are to be returned

  • No charge
