Adult System of Care

Fresno County’s Adult System of Care provides an array of services to Transition Aged Youth (TAY) 18-25, as well as adults 18 years of age and up throughout their life span.

Below are ways individuals may access care or to enter the system of care. The list below is not a comprehensive list of services, but ones that individuals can use to begin accessing care or can start the process by calling the 24/7 Access Line at 1-800-654-3937.

Levels of Care Adult Services

Services Levels in our system of care are used to help identify the right resources and services to assist in achieving wellness and recovery. Our levels of care are Low Level, Intermediary/Medium and Intensive-High. These services range from open access, self-referral programs that are low intensity, to medium level and on to higher intensity and level of care.

The various service levels have different criteria and qualifications, as well as programs which may be court order and require a specific referral. Part of the access process is to work with the individual to identify what services are needed based on their current wellness challenges.

Adult Individuals, families, providers, and others seeking to support individuals with a possible behavioral health challenge (mental health or substance use), may seek care and services through some of the following options:

Anyone may call the 24/7 Access line at 1-800-654-3937 for information on services, screening, referral, and linkage and/or eligibility information for various services.
Individuals and family may seek services through calling the Urgent Care Wellness Center M-F 8:00 AM to 5:00 PM at (559) 600-9171 or walking into the Urgent Care Wellness Center located at 4441 E. Kings Canyon Road Fresno, CA 93702.
Individuals who may be experiencing crisis may access the Crisis Stabilization Center on their own 24/7 by calling (559) 453-1008 or walking in at 4411 East Kings Canyon Road Fresno, CA 93702.
Assisted Outpatient Treatment (AOT) provides community-based support and court oversight of mandatory outpatient treatment by a civil, not a criminal, court and is dedicated specifically to adults with severe mental illness. For additional information on AOT including referral criteria and form, please visit the Fresno County DBH AOT webpage.
The CARE Act introduces a new civil court process that provides community-based mental health services to eligible individuals. It prioritizes treatment, housing, and community support. CareActFresno.com

Also, the Crisis Residential Treatment Facility for Adults provides 24/7 support to those needing crisis mental health services, crisis assessment, stabilization, and inpatient services. The CRT as it’s known provides a positive, temporary alternative for Fresno County residents experiencing a serious psychiatric episode or intense emotional distress who might otherwise face voluntary or involuntary hospitalization and/or incarceration. The Crisis Residential for Adults can be reached at (559) 860-4422 and is located at 496 S. Barton Ave.

Individuals seeking care and services may also utilize the County’s Provider Directory. Please access the Provider Directory which is a listing of all Fresno County Mental Health Plan operators, both county operated and providers contracted with the county. The Directory is available in the county’s threshold languages (English, Spanish and Hmong).

Older Adult Services - may be accessed through the Access Line or contacting the older adult team at (559) 600-5755 for more information M-F 8am to 5pm.
Perinatal Wellness Center - 142 E. Cesar Chavez Blvd., Fresno, CA 93706, which uses a multidisciplinary team provides mental health services to pregnant and postpartum mothers and their babies, as well as to fathers struggling with paternal postnatal depression through in-home or office setting. Individuals may self-refer by calling (559) 600-1033 or calling the Access Line.
Fresno Metro Area Outpatient Clinic - which also provides an array of services may be contacted through the 24/7 Access Line or directly M-F 8am to 5pm at (559) 600-4099 for information, or persons may walk into the Urgent Care Wellness Center M-F 8am to 6pm.
First Onset/TAY Team - Provides an array of more intensive behavioral health services to persons 18-25 (often referred to as transition aged youth (TAY)), Interested individuals can self-refer by walking in M-F 8am to 5pm to 2719 N. Air Fresno Dr. Ste. 107 Fresno, CA 93727 or can you call (559) 600-9088.
Public LPS Conservatorship - If you are needing information on conservatorship for adult individuals who require psychological and/or psychiatric services and care coordination as being impaired by their condition where they cannot provide for their own basic needs as determined by court please call (559) 600-1500 M-F 8am to 5pm for more information and assistance.
Public Guardian - for general information about the public guardian’s office, probate conservatorships or other related questions, you can call (559) 600-1500.

As Fresno County Department of Behavioral Health operates numerous programs and funds a number of community programs in the metro Fresno area, as well as our mountain and rural communities, we are unable to list them all.
